Important results from the project
The aim of the project is to develop and test a service design-based model to enable person-centered care by changing the care flow for care providers, patients and relatives. The long-term goal for the project is to create possibilities for self-care within each care process and enable patient involvement based on needs, capabilities and wishes.
Expected long term effects
The project has identified solutions to meet patients’ and their relatives’ needs in order to enable an active partnership between patients and health care organizations. With an integrated care flow, which is designed with and for patients, all involved parties are benefited. The patient can follow, participate actively in and manage his or her own health, relatives can more easily support the patient and health care professionals get tools and methods to involve the patient.
Approach and implementation
In order to create person-centered health care with seamless care flows, an understanding of the situation, wishes and needs of the patient is required. In Step 1, the project tested service design methodology to investigate the needs of heart failure patients and patients admitted to an infectious clinic. This has been done through interviews and surveys with patients, relatives and health care professionals. Subsequently, solutions have been generated by the project, patients, relatives and employees to meet the needs.
