Purpose and goal
To examine, develop and implement a model of individualized care based on the biorhythm. Goal partly met. A further development is needed in terms of working methods, organization and technology. 39 investigations have been initiated. Four were closed due to technical problems and seven because of users deteriorating health condition. The project has led to registrations in the Senior Alert incontinence module. 60% have got adjusted night visits in time and number. Five users now go to the toilet. 35% noted reduced anxiety and better deep sleep at night. Three are difficult to assess.
Expected results and effects
The Sensor methods to obtain each individual´s biorhythm and individualize the care have increased the staff´s understanding that every client is unique, with unique needs. Support to the toilet in time instead of using incontinence pads, if possible. Five people go to toilet. Awareness about getting the right size for incontinence pads has increased. Three people don´t need night visits any longer. Many patients sleep better, are more alert during the day and eat better. The work method requires good infrastructure in the buildings - a problem that we solved with tablets with SIM cards.
Planned approach and implementation
The project run by a coordinator consisted of 6 WP. The project assistant supported technical and practical tasks. The investigations have been conducted in both home care and residential care, geographically spread with different managers with their own budget. Selection of users has been done by staff based on given criteria. Before the start, information and- training sessions were held, consent collected and manuals developed. The investigation period was set to 4+4 weeks, which mostly were delayed. Every care unit needs to have a designated coordinator with an explicit mandate.
