Purpose and goal

The goal is to identify, implement and disseminate good ideas about how e- health can be used for new and innovative work processes for the group multi-sick the elderly. This is achieved by making it easier for both care providers and providers to meet and collaborate in a functioning ecosystem for e- health. The purpose is to make it easier for patients to participate in their own care, increase security and reduce the need for hospitalization.

Expected results and effects

A current situation was described with regard to the introduction of self-monitoring and an ecosystem for e-health with its actors. Needs and process mapping have described what self-monitoring can look like and which parts should be included in an introduction. This has been the basis for the introduction and innovation procurement. The introduction of monitoring has started and gathered its experiences in a report. Experiences have been actively disseminated continuously. Effects of reduced hospitalization etc. have not been seen yet due to pandemic.

Planned approach and implementation

The implementation of the project was done through three work packages according to plan: Mapping the current state of knowledge, Definition and establishment of an ecosystem for e- health and Implementation guide for e- health. The project was extended in two rounds when the Corona pandemic made contact and participation with the care activities more difficult. Challenges have been to show that individuals must change their way of working, ownership of the change and that the need exists now. Participation has been high from employees and patients.
