Purpose and goal

The project goal of establishing Proof of Concept (PoC) for a patient-centered care model supported by mobile health has been achieved through implementation of applications and workflows in a hospital where healthcare staff are now working with the new model. Interim targets for the clinic were to streamline work and improve patient safety, which the project reached through secure communications between caregiver and patient, and internally for staff. Other interim targets to increase patient benefits were also met as patient involvement and support at home increased.

Results and expected effects

Data transmission, communication and insulin calculations happen in a secure patient manner through apps for patients and healthcare staff. A new model and procedures have been developed for staff and patients using digital tools. Staff saved time and had more effective contact with patients and internally, especially when doctors were on call at home. The patient app, with the aim to make the patient more independent, supports education and simplifies daily life at school and leisure. A safer contact happens between patients, relatives and other carers.

Approach and implementation

A training program for staff was conducted via the web and face-to-face sessions. Evaluation showed that training was very valuable and necessary. The new approach was implemented iteratively in a controlled trial-and-error fashion, where the apps were first used by people who have become expert users and could identify important issues. One lesson was the importance of testing the new approach in real practice. Staff and families´ experiences were studied by interviews and surveys over a few months during and after the introduction.
