Purpose and goal

The goal and purpose of this project has been to contribute to: - the development of goals for specialized self-care as a natural part of the Health care sector -patients should have access to effective treatment that the patient, if the patient wants, can operate on its own as much as possible no matter were the patient is ie at home. The goal is to obtain a high quality of life and the best possible health -Increace the knowledge about how medical devices can enable a more individualized care.

Expected results and effects

One obvious result is that a working group was formed by several of the relevant stakeholders. The working group are planning to work with reducing identified barriers and increase the knowledge about the area. Increased collaboration is achieved by collaboration between patient organizations, professional organizations, health care representatives and industry in the continuing work of the working group with specific projects.

Planned approach and implementation

The round table discussion and the meetings before that have focused on the challenges and demands that the transition to a more distributed patient centered care brings. Companies, local governments, staff and patients are affected by these challenges and how we address them. The results from the project have been presented in a report that has been spread to stakeholders that didn´t take an active part in the work. The project only focused on specialized self-care/home care where the patient (or relative) itself takes a great responsibility for their care.

The project description has been provided by the project members themselves and the text has not been looked at by our editors.
