Important results from the project
The purpose of the project has been to connect patients, municipalities and regions digitally via an IoT platform. The parties have been able to continuously collect and share information, thereby creating the basis for an ongoing continuous working method that includes the patient, relatives, municipality and region. The goal of the IoT platform has been to increase continuity for the patients and contribute to creating greater control over their own health, and through control over the patients´ vital parameters create opportunities to work more preventively.
Expected long term effects
The benefits of using the IoT platform have been that the focus has shifted from treatment to preventive efforts. The patients have been able to take a more active role and gain greater control over their illness. The possibilities to follow up vital parameters more frequently have contributed to increased security for the patients. Seen from a medical perspective, the video calls have created the conditions for making more accurate medical assessments and the ability to continuously measure vital parameters have contributed to better control over the course of the patient´s illness.
Approach and implementation
The project was delayed at the start because the municipality´s data protection officer came in at a late stage. This led to that the work of creating new work routines and work processes across principal boundaries was delayed. However, the nurses in the municipality started the work with home monitoring before the doctors in the region were involved. When the legal parts were in place, it went relatively fast to create working routines thanks to the already established well-functioning cooperation between staff in the municipality and the staff at the health centre.
