Purpose and goal

The purpose of this project was to analyze possible and testable solutions to improve self-care in patients with heart failure. Of 24 identified solutions, five have been selected; optilogg, My health plan / video calls, Physical kits, Information pages on the web and learning coffee shops. A plan for pilot testing and resource analysis has been developed. The project lasted for seven months.

Results and expected effects

The project identified 24 possible solutions based on the needs expressed by patients and caregivers. Of these, about 50% was proposed by patients / relatives or caregivers. The identified solutions were expected to meet 39 of the 58 formulated areas of needs from patients and caregivers. After the selection had been made based on value, time and resources five solutions remained. Recommended choices is to conduct a pilot test of all five solutions from 1 nov 2014-31 april 2015th

Approach and implementation

The opportunity analysis has acted as a sub-project in which the results from the previous needs-assessment where used and where deliveries will be input into the subsequent test phase. The project organization consisted of a multidisciplinary project team. The work was led by a development leader of the county council´s Development and Innovation Unit, Qulturum. Other participants were experts in the field of Purchasing, IT, Health care systems, Patient Experience, Research, and Medicine.
