Purpose and goal

The aim of this project was to increase a patient’s ability to take part in their own sick leave process through a process based on the needs of the patient and with the support of a specialised team and a digital communication platform. The new way of working in specialised teams is implemented at 2 primary care units and Rehabresan is now a working digital tool that works in different interfaces and with some coordinating functions.

Expected results and effects

The specialised team with dedicated doctors increased patients experience of continuity and a holistic view on their problems. The absence from work for the patients got shorter and more often the it was only part time. The platform development has taken GDPR in consideration and is adapted to this new legislation. Our evaluation shows that Rehabresan facilitates the process for the participators we tested although there are still room for improvement. We also have a plan for a broader launch.

Planned approach and implementation

We have described the processes for the most common diagnoses. The dedicated team has developed new procedures for the patients where the coordinator has an important role. We developed prototypes for users to test and after testing we developed a communication platform built on Dreamlers technology. The platform is now in use with 10 patients at a primary care facility testing it. We have prepared for a launch through internal meetings, meetings with local stakeholders and with a large private caregiver in Sweden.
