Purpose and goal
The project aims to enhance health, independence, and quality of life for older adults living at home. In close collaboration with care staff and elderly users, we will develop a tailored version of Healthfinder – a CE-marked digital tool that visualises lifestyle factors, strengthens self-care, and fosters meaningful dialogue between the user and care providers.
Expected effects and result
The project will deliver an adapted version of Healthfinder for older adults in home-based care and their care-givers that is technically functional, user-tested and congruent with existing care pathways. Anticipated effects include trackable metrics around risk factors and determinants of health that supports better care focus and communication between end-users and caregivers, and early risk detection. This supports a more proactive, person-centred and sustainable elderly care system.
Planned approach and implementation
The project is carried out in three work packages. 1) Needs assessment conducted via older adults, care-givers and healthcare providers 2) Development of adapted version of Healthfinder 3) Pilot testing to evaluate impact for end-users, and care-givers.
