Important results from the project

The main purpose has been to develop a network for cooperation för patient-centerad care where the individuals needs are met jointly. The main goal was to establish a vision, goals and an action plan that the network could use for further development. The vision adresses challenges, possibilities and the desired outcome in year 2030. The network is now ready to start the work with realising the vision and goals by using the action plan forward.

Expected long term effects

The present network was complemented by external actors, outside the healthcare sector, in order to use the joint innovational strength and competences in society by using a larger base for innovations, resources, knowledge and experience. This in order to reach the goals of creating preconditions for a large and dynamic networking within patient-centred care. This network is now continuing its work to create and fulfil the preconditions and put them into action and realization.

Approach and implementation

The project has been managed by a mulitdisciplinary team, based on the e-healthnetwork in Region Östergötland, with a user- and design driven mindset. The projects model by working in six workpackages has served the purpose well with a thorough support from stakeholders, a well formulated plan and activities that has created value for the overall work as well as inbetween packages. A continous dialogue between stakeholders as also created possibilites for adjustments and complements when needed in order to reach further in the work.
