Testbed for integrated health and social care for the vulnerable geriatric through the Norrtälje model

Purpose and goal

The purpose is to create an innovation platform from a citizen-based perspective, with the focus on the frail elderly, as this group of people are the most challenging to the effectiveness of wellbeing and care delivery. Achieving better solutions requires involvement of all actors around the target group. We have mobilised all actors and levels the municipal and regional actors; the political level; private and public providers. We have begun to establish an environment where the actors jointly develop their capability to test better solutions and challenge conventional thinking.

Expected results and effects

All actors, professions and decision levels have followed the journey of the inhabitant through the fragmented system. Collectively we see what no one can see on their own: how the system factors prevent us from achieving the purpose; what the real problems are; and the opportunities for higher effectiveness. We have created freedom for the right expertise the professionals closest to the inhabitant to drive innovation, test new solutions and challenge the hinders. Established support from the political and top-management levels for continued systemic innovation work.

Planned approach and implementation

The ambition has required extensive anchoring and mobilisation to establish a common understanding of (1) how the system works today, (2) the need for systemic innovation and (3) the need for freedom for the professionals to pursue innovation work. Built ability in a team of professionals from all actors, to understand the individuals for whom the system exists, rethink and test better solutions in a real user environment. The approach has led to concrete solutions and built the ability to drive innovation at the system level.
