Providing care remotely in local environment in Skåne - an innovation in process and organization development

Purpose and goal

The aim is to build knowledge and competence within the health care organization with regard to chronic disease and the care of those patients in their homes. Main focus is on the organization and how to build a team which in an effective way utilizes eHealth care solutions. In this project we focus on changing the care for chronic COPD patients and create a support function for them called ´eCall center´ that has the ability to accept all forms of patient data, app data, signaling data, video links and other electronic data signals.

Expected results and effects

An increased spreading of Best practice by making expertise available to a larger group, as well as support to organizations when it comes to new technology. Utilizing new technology within the home of patients opens up for better care, availability of health care professionals and safety for the patients.
