Purpose and goal
The purpose of the preparatory project is to identify needs and improvements for the target groups regarding health and care that takes place in collaboration between the municipal and regional care.
The goal is that the frail elderly and foreign-born people will receive an accessible and understandable service in one building where the hub of health and care is located. The hub will provide collaboration between the actors so that the individual is easily guided through the complex system.
Expected effects and result
The expected result is that we have developed a good basis for how we can innovatively work further with a coherent healthcare system for the target groups. The project has received relevant input from the target groups that can be used in practice to support a coherent healthcare system.
Planned approach and implementation
We plan to conduct interviews with people from the target groups to gain an understanding of what they perceive as obstacles and which needs should be prioritized. Interviews with employees from the region and the municipality will also be conducted to get their views and experiences.
Mapping of the current situation, monitoring of the external environment and study visits are planned.
