Purpose and goal

CONNECT-CARE aims to develop and implement an innovative and interactive platform to empower children and adolescents in managing their health and adopting healthier lifestyles. The envisioned platform will go beyond traditional prevention strategies by reshaping the way young individuals access and engage with healthcare systems. With the use of an AI-assisted personalized obesity management approach, the project aims to transform how young individuals receive and perceive health messages.

Expected effects and result

CONNECT-CARE will shift healthcare delivery towards more patient-centered and digitally-enabled models, transforming daily health and care, and ultimately leading to better health outcomes. By emphasizing prevention and leveraging tools and data, the project will improve the quality of obesity care, making it more cost-effective and personalized. Additionally, it will enhance healthcare access for users while simultaneously improving service delivery for healthcare providers.

Planned approach and implementation

The project will use data from primary healthcare systems, covering clinical, socioeconomic, behavioural, and cultural aspects, to develop a pan-European Youth Empowerment Hub (YEH) with digital tools, including the personalized obesity management platform (NutriFit Advisor) and an educational platform. The YEH will integrate into the healthcare system for bidirectional interaction between users and health professionals, enhancing healthcare providers´ capacity to deliver services efficiently.
