Important results from the project
Purpose with the project is to develop and implement home care with support of distance monitoring. The overall goal of the feasibility study was to understand how the concept of can be developed with the ultimate goal of reaching regional implementation. Identified issues: Which patients need to be monitored, how and when? Distribution of responsibilities across organizational boundaries? Centralized vs. local monitoring? Socio-technical aspects? How do technical solutions meet eligibility requirements on data collection, storage and visualization?
Expected long term effects
The feasability study has delivered patient journeys and proposals for pilot studies that can clarify the possibilities and needs of distance monitoring and collaboration across organizational boundaries. Södertälje Hospital´s examination of the market in terms of technical solutions for distance monitoring has been followed. These are often more focused on less ill patients and self-care. Identified challenges: create better measurement tools, ensure access to measurement data, take into account socio-technical aspects, reduce the cost of licensing fees, etc.
Approach and implementation
The preliminary study has been organized into four work packages focusing on meetings and administration, organization and processes, pilots and technologies. Workshops, observations, interviews etc. have been conducted. The project group with participants from Karolinska, SöS and SLSO has met every two to four weeks. The steering group met every two months.
