Important results from the project

The project has worked towards four objectives: to introduce a collaboration platform with traceability, digitize existing processes with respect to privacy and GDPR, and implement new, more efficient and secure working methods. Plattformen Collabodoc was introduced and has been extended for planning of broad implementation. A comprehensive DPIA was carried out. Overall, the project has streamlined collaboration, strengthened patient safety and reduced risks for the individual.

Expected long term effects

In the long term, the project is expected to contribute to a more cohesive and secure collaboration between the affected actors. Digital and standardized working methods create better conditions for information sharing, increased accessibility and more efficient processes. This strengthens patient safety, reduces the risk of errors and contributes to more equitable, person-centered and sustainable care and welfare in line with the transition to good and close care.

Approach and implementation

The project was implemented in phases, where digital collaboration was gradually introduced. Initially, the municipality´s HSL staff and health centers were included, and then assistant nurses in home care and support assistants in LSS were included. This enabled adaptation to the needs of the activities. The project developed mainly according to plan, although some elements, mainly related to information security and GDPR, required more time. The collaboration worked well between the parties.
