Purpose and goal
Overall goal: - Correct help and support at the early stage of childrens´ life. Project objective: - Well-functioning, systematic information exchange through interdiciplinary assessment of needs. - Fewer verbal retakes for children and their guardians when seeking help. The purpose is to create a safe and secure upbringing for each child through supporting and coordinated efforts at an early stage of the child´s life. The main perspective and starting point should be the child´s needs, not repertoirs derived from pre-established organizational structures.
Expected results and effects
Families, children and adolescents in Ystad have access to help and support at an early stage and know where to turn to. Consensus on working methods, information sharing and efforts that are based on children´s/adolescents´ needs. Different agents have well-established, well-known commitments. They cooperate actively, through extended responsibilities that overlap one another. Controlling systems provide support and incentives to find innovative working methods in order to hand children and caregivers help and support at an early stage.
Planned approach and implementation
The project´s target groups and co-creators are children, adolescents and custodians. A plan to achieve correct help and support is established through gathering shared and necessary information. The commitments of family coordinators are defined by examining which support is important to achieve help and support. Responsibilities and working methods will be derived from the outcome of those conclusions. The objective is co-ordination that is unhindered of organizational boundaries. Through examination of parlance and approach, an assessment terminology will be designed.
