Purpose and goal

The aim is to create a sustainable and equitable healthcare structure in rural areas by developing a mobile team and a shared platform for complex healthcare needs in the home. Impact goals: - Better accessibility and continuity for patients - More equitable healthcare regardless of coordination ability - Reduced burden on inpatient and primary care - Improved work environment and attractiveness of local healthcare

Expected effects and result

Shared view of the current situation, obstacles, and development needs in collaboration on complex care needs and home care. Collaboration model for how the region and municipality can work together on home care, with a focus on self-monitoring and information flows. Scalable implementation plan for introducing the collaboration model in the region and municipalities, including assessment of effects on care place availability, staffing needs, and patient benefits.

Planned approach and implementation

2512–2601: Mapping of care flows, obstacles, roles, and resources; workshop with patients/users; report on the current situation. 2602–2603: Co-creation workshops to develop a common model for collaboration in the home; modeling of care flows and digital solutions; prototype for a collaboration model. 2604–2605: Development of business case and analysis of effects, anchoring in regional and municipal management, plan for scaling up to more municipalities and national dissemination.
