Purpose and goal

The aim of PDU, Person-centered Documentation and follow-up is to enable co-creation when designing goals, and follow efforts and results for the individual and at the aggregated level for the organization. PDU has been developed according to the use of ICF and to visualize abilities and goals as well as to enable structured visibility of efforts according to KSI. A manual has been designed for implementation of PDU in care. This enable PDU as a concept makes IT possible to spread to more users.

Expected results and effects

PDU-IT makes it possible to describe the individual´s functional abilities based on values and the individual´s own superior goals. PDU has been supplemented with KSI, which enable follow-up and visualizing of efforts. PDU has been further developed for use in a unit for habilitation as well as in a home for children with autism. The implementation process has been documented and described to allow dissemination to other units. The use of PDU leads to a way of work that is based on the individual´s needs and where the goals, efforts and follow-up are supported and visualized by PDU-IT.

Planned approach and implementation

When PDU was implemented, education and dialogue took place in workshops with focus on collegial learning under supervision from a coach with knowledge of PDU both the working method and PDU-IT. In addition to the workshop, a dialogue with those who developed PDU- IT took place continuously with communication about suggestions for development and change. This collaboration together in the project team have developed. The team will continue the work together.
