Purpose and goal
The ambition of Forum for Health Development is to show examples of concrete improvements in health care. Forum for Health Development also want to contribute to a deeper dialogue cross all relevant stakeholders in the sector about the true challenges and solutions we are facing.
Expected results and effects
During the project, all goals have been reached. With completed pilots in prevention, integrated care and patient-centered technology solutions we have showed specific areas of improvement in the Swedish healthcare. In parallel, we have, with the pilots as a starting point, contributed to a deepening of the dialogue through hosting a major conference for key decision makers in health care, released three well received reports, four influential debate articles and several seminars.
Planned approach and implementation
FFV has conducted four pilots: 1. Prevention of diabetes / cardiovascular disease: Individuals in Södertälje at high risk of diabetes / cardiovascular diseases were identified 2. Vertically integrated care Four primary care centers in Södertälje gave a group of complex patients a joint assessment with GP and hospital specialist 3. APPlicera hälsa The APPlicera hälsa competition focused on developing new patient-centered technology solutions 4. Patient-centered technology solutions, Aleris Bollnäs The pilot tested digital solutions for better care of patient over 65 years
