Important results from the project
The aim of the project was to demonstrate that patients could experience improved quality of life by being managed through a telemedicine solution. The new role ´health coordinator´ would add both to increased quality of life for the patiensa as well as to relieve pressure from the clinic. The solution that was developed should have the potential to manage COPD patients efficiently but it should also mean that in the future other chronic diseases could be well managed. The purpose of the project was well fulfilled which is proven by the great appreciation from the patients.
Expected long term effects
The long term objectives of the project are increased cost of care and quality of life for patients who increasingly will be cared for at home. The project has had a major impact on the participating companies who have realized the potential of care at a distance, and the solution is the basis for several of the companies´ new strategies. For the Lung clinics, the result achieved by the project means new opportunities for getting help to effectively manage patients who are outside the hospital. The interaction between the health coordinator and the clinic will relief the clinic.
Approach and implementation
The project has been designed as an integration project towards a total solution. The integration of co-products from partners has gradually increased towards a final solution. During the gradual integration, we have implemented four demonstrations where medical staff showed how they manage patients across the system. To provide a total solution, we have had to deal with 14 identified challenge areas which were processed in six work packages. Work Package Leaders have biweekly coordinated the project in team meetings. We have had 49 project team meetings during the project.
