Important results from the project
The project has strengthened collaboration within Lidingö social services and Region Stockholm, which has led to joint a proposal with CSES, NKA and SKR within SustainGov. Interviews with relatives have provided important insights into early support. Development areas include longterm relationships, confidentiality management and a holistic approach. Collaboration has been established with CSES, NKA, SKR, Socialstyrelsen and researchers for continued qualitative support to the target group.
Expected long term effects
If the project partners continue to develop the work in a common direction, it can provide valuable lessons for both the municipality and other actors. Results and working methods can be spread to more municipalities and regions. Through increased collaboration, the project can also contribute to a more effective change in working methods in line with the comorbidity reform that the government and authorities have initiated in 2025.
Approach and implementation
The project was carried out in five phases, some of which were carried out in parallel: 1: Document studies, targeted interviews and surveys 2: Review of literature and research, case analysis and interviews with relatives 3: Two workshops with representatives from Lidingö stad, CSES and a researcher. A coordination dialogue was also carried out in which Lidingö stad and the Region participated 4: Analysis of previous phases 5: Report writing and compilation of recommendations for further work
