Purpose and goal

The project aims to develop and test shared models and frameworks for hospital-at-home care, adapted to Swedish and Nordic conditions. Four university hospitals with established initiatives form the core of the project, complemented by two rural regions to reflect varying conditions. The goal is to enable large-scale adoption by translating analysis into practice. Broad collaboration will create a platform for scalable, patient-centered and integrated solutions.

Expected effects and result

The project strengthens knowledge, coordination, and practical implementation of hospital-at-home care. By bringing together healthcare providers, medtech, academia, and patients, it creates shared frameworks and guidance for procurement, technology, and organization. This promotes equitable access to care, drives innovation, influences policy, and enables scalable models in both urban and rural settings.

Planned approach and implementation

Four university hospitals and two rural regions co-develop and test models for hospital-at-home care. The project is research-led and involves collaboration with medtech, academia, patients, and caregivers. Through case studies, tool development, and policy work, shared frameworks are created. Solutions are tested and adapted in diverse contexts to enable national and Nordic-scale implementation.
