Better health with the patient in the driver´s seat - Integrated care 2.0 w. digital tools & organisational makeovers
Purpose and goal
The purpose of this project is to create the conditions for the development of broad and dynamic collaboration in integrated care and people powered health where healthcare and social care of the individual is perceived and functions as a whole. The aim is to develop and anchor a vision with the related sub-goals and a plan for how an innovation environment will be established in Östergötland and further external environment monitoring will take place in order to identify needs and conditions
Expected results and effects
The existing network is extended to include other external parties actors outside the healthcare sector in order to take a greater part of the overall innovation power in society in a faster and better way. Other actors in society, such as academia, industry and the third sector, as well as other relevant authorities, are mobilized in the work to build a well-functioning whole from the perspective of the individual. A bigger innovation base in the form of new resources, new knowledge and new experiences creates the conditions for cross-fertilization and faster / better development.
Planned approach and implementation
The work will be led by a multidisciplinary project team. A user- and design-driven approach will permeate the project. In addition to drawing up a vision with associated sub-goals, the focus will be placed on a plan for how to establish the environment in Östergötland. As a final activity, there is a national knowledge seminar with researchers, practitioners and industry where the project´s results are presented and discussed. Results will be communicated continuously through the project website and spread openly to stimulate broad collaboration far beyond the project team.
