Call Impact Innovation: Implementation project for a reformed public sector

Purpose and goal

The project aims to strengthen patients’ role in their own care through digital tools, new workflows and collaboration between regions. Its goal is to develop a joint model for heart failure self-monitoring at three levels – from basic support to advanced remote monitoring – as a step towards a future national model for proactive, equitable and resource-efficient care. The work unites healthcare, academia, technology and patient organisations.

Expected effects and result

The project is expected to increase patients’ confidence, self-management and quality of life while reducing readmissions and hospital stays. For healthcare staff, it provides clearer workflows, improved decision support and reduced workload. At system level, it promotes efficient use of resources, equity and stronger patient empowerment. Results will be shared through SKR’s networks and SustainGov’s learning processes.

Planned approach and implementation

The project is structured into five work packages involving healthcare, academia, technology companies, policy actors and patient organisations. Implementation and testing take place in Skåne at SUS and Trelleborg Hospital, while Region Halland contributes experience from primary care and basic self-monitoring. Results are evaluated scientifically and disseminated nationally via SKR networks, SustainGov and shared learning activities.
